In the case of this comic and the context, it’s especially awkward.  
The night before, Twilight finally became comfortable with calling herself ‘mama’ to Spike. Before then, she very noticeably avoided using that word, Moondancer even pointed it out directly.
 
There are moments where Twilight assumes her caretaking of Spike is temporary until someone more suitable either assists her, or adopts him outright.
 
When Twilight called herself Spike’s mom, it meant a lot more than a title. She was reminded in that moment that she IS Spike’s mom, and how important their bond is.
 
I’m sure Shining Armor meant not harm, he was unaware of their moment.  
(In fact, Twilight tries to teach Spike to say ‘Twilight’ much earlier on, but has no luck)
 
The fact that Spike was able to learn to say Twilight so easily makes her doubt everything again. Does Spike not actually see Twilight as his mom anymore? Before, she may not have minded, but after their moment of bonding, she thought she discovered who she was, only for more clouds of doubt to roll in.
 
Sure, Spike was definitely going to out grow saying ‘mama’, but this isn’t how or when Twilight expected it to happen. Spike is still a toddler, though he’s smart, how much of what he says is defined by logic or instinct is also unclear.
 
As for how Twilight deals with this, well, you’ll see soon.
