Deadline or not we now have to find an in-universe reason for these confusions especailly since they used this background in two separate episodes five seasons apart.
Example the pet size weight is still in the newer version despite the lollipops have been updated in look.

Keep in mind that the “ponies” in the show are only equine-like… whatever species they are, they not horses or ponies as we know them. The split toe could be a nod as to how they are able to lift and carry things. The real answer of course is that these were all background details that the show’s staff didn’t have time to quibble about while trying to meet impossible deadlines.
Plus Twilight looks rather… oversized especailly when compared to Spike who is supposed to be taller than her at this point in the episode.

Plus it never stated that the room Spike is being examined in is in a separate place from this picture. For all we know it could be just two different rooms in the same building.

Hmm, this doesn’t disproven the fact that hoof is not a pony’s hoof. If anything this shows that the vet and doctor use the same office. In which case, all mysteries example, like having lollipops near a pet size weight and other confusions are solved.

You got it wrong, this is the doctors office, there’s even another picture of pony anatomy behind Spike. The same one they took Zecora to when she was sick.

Also, the hoof as stated in a separate comment is very not of a pony since it has two toes. As for the suckers I have no idea what they are doing there.

Nope, rewatch the episode, it’s very much a veterinarian office. The Doctor herself even clarifies this when she states that Spike is not a dog so she can’t help them.

This isn’t the veterinarian’s office. It’s the pediatrician’s, and presumably the chart is for a foal.
That’s putting a lot of assumptions forth especailly when you realize how pointless it would be to even have a graph for ponies in a place where pets are worked on. After all, a dog’s skeletal structures and organ placement in the real world is very different from a pony. Plus wouldn’t it make more sense to have three different graphs, one for each species of pony, which each graph specializing in certain areas for each pony?
Example: a unicorn graph would have extra attention on the horn since it might be very similar to a rhino’s horn and that damage to it could be very dangerous while a pegasus graph would give more attention to the wings since without those a pegasus would surely die but just simply leaving their home.
And really even if they had three graphs in there it would still be rather pointless in a veterinarian office.
The figure seems to have the same odd curved snout that the rather un-equine ponies in this show have. Compare to Twilight sitting there.
