It’s the opponent who loses 5 life and the caster who gains 5 life.

makes no sense to loose 5 life and then gain 5 life except it happens in two different phases. like “gain 5 life at the end of turn”. within a phase, it doesn’t matter if you get below 1 health if you are at the end of the phase above 0.
