I was looking at it wrong

What he’s actually doing is nodding his head closer to her but if you look at it in a different way it kind of looks like he completely turns to face her and we’re looking at the right side of his face and he looks like his face has turned into a pudgy old man

His dorsal fin things in the second panel look like really ugly cheeks from an old man looking off to the side. It’s like one of those optical illusion face images. He has two faces.
