depending on the nature of the coma your brain could be quite active, albeit in very unproductive ways, and if you haven’t regressed into a vegitative state then it’s probably that type of coma. no, it’s the rest of your body that would have atrophied: muscles, digestive system, bone strength, epidermis… there are pretty terrible consequences for lying in a bed for 20 years without moving. that is, if your family can and wants to afford to care for you for that long.
 
but hey, that’s only true in the weird coma dream reality. maybe in the real reality they can keep ponies perfectly healthy for extended periods of unconsciousness!

If you’ve been in a coma for 20 years, your brain will have atrophied too far for you to be considered you in any meaningful way.  
What’s left of you is barely even alive at that point, good only for its transplantable organs.
