Well, your eyes automatically do, initially, for your protection. Your preferences will redirect them from there until you decide to consciously control them. Most people don’t notice their own focus shifts unless directed to by someone else.
 
Since as the artist, you control the focus of the image, people are likely to complain if the one you chose is different from the subconscious default. Additionally, a shift in focus as illustrated in your picture is liable to happen only if the person pictured was life-sized.
 
Which don’t get me wrong, would be totally awesome. But for people with more conventional sized displays, this ironically breaks immersion even though by practical understanding it should enhance it.

Eyes and camera lenses do not automatically focus on the closest object, they focus on what you yourself/the cameraman wants to focus on.  
And there wouldn’t be much point to uploading an image where everything but her hand is blurred out…:v

I think this was an effort at making the image 3D, but usually when somebody reaches for you, your focus is on their hands. Even if they’re really hot.
