On second evaluation, the lack of bulge is probably due to the genital slit he seems to have in other art. The “breasts” are very likely just the curves of his body, again probably to accentuate femininity.
All that considered, he’s just a femboy that at first glance has a couple attributes that make him look more girly than most. Perhaps that ruins the appeal for some, but there you go.
