Equestria has never been referred to as a world, always and only as a nation. Somnambula is canonically inside of Equestria while Saddle Arabia has been referred to as its own separate nation.

Possibly. Mistmane’s hometown looked very Asian. And there are other locations based on areas not based on America and the UK like Somnambula (which appears to be based on Egypt) and Saddle Arabia (which is obviously based on Arabia).

I haven’t but this episode doesn’t take place in the Pony equivalent of Japan (also Japan makes some of those pillows for adults too).

While you’re right that there probably are actual official MLP pillows aimed at children (I once had a Simba pillow that Disney officially made), I still think this is one of those adult jokes.
 
They’re at a fan convention (not at a store), the ponies there at the convention are adults, the image of Daring Do on the pillow isn’t a normal one but instead one of her tied up (aka a bondage joke), and Rainbow Dash’s reaction makes it seem like it wasn’t a normal pillow. Come on. It’s not rocket science. It’s definitely a little meta-joke that was made for adult fans in mind. It’s subtle enough to fly over kids heads but it’s explicit enough for adults to get the joke and find amusing.

This is one You Know For kids moment that actually is for kids.
 
People think this is a innuendo when in reality it’s a marketing campaign to get children to buy MLP pillows. X3

Well yeah, that show was also aimed at a slightly older demogramphic too. Shows aimed at older children can get away with a lot more than shows aimed at younger children. Look at the more recent Transformer cartoons, the two Avatar series, and Gravity Falls for more modern examples of this in action.

It’s possible Hasbro took the Animaniacs approach, and had something more lewd/blatant that was guaranteed to be cut by the censors to make the pillow seem more tame by comparison.
