Thats exactly it. In this AU Fluttersh is in love with her sister because of a childhood trauma where punk Flutters came and rescued her. Now she will do anything for her and her sister knows it so she uses Fluttershy a lot.

I guess Flutters really loves her sister.
She’s saying that her twin sister slit her cheek and arm and made her get a tattoo for this one drug operation she was a part of.
Which is a pretty fucked-up thing to do to your sister, even if you do need a stand-in.
Honestly, temporary tattoos of the scars and tattoo might’ve worked just as well, depending on the exact situation.
