Nope, this scenario focuses to Celestia’s pregnancy, check the listed shipping tag and refer to the source, the fault goes to the OP who found that image and uploaded to here…
OP, please, that artist have inserted a story from this image’s original source. Why skipping on inserting that to here? It’s worth reading.
That artist is also a genius, where they let Twilight calling Celestia by the nickname, Cely. By the way, I have to clean up some things in that story, because it’s not on fanfic standards, the artist went on a different way on writing, which I guess I’m totally fine for them if they want to take that route anyway.
“They should call me the Princess of uselessness.” Celestia said with an usual weak and a lack of self-confidence.
“Cely… I can’t let you say those cruel things about yourself!”
“I can raise the sun, but I can’t have babies! Isn’t that pathetic?!” She said while turning her back from Twilight.
“This is not pathetic, it can happen to everypony and you know it. But… I’m sure that we can find an option, maybe Discord can help, or… I don’t know… We can adopt?” Said Twilight with a little smile full of sympathy.
“Discord can’t do anything about that… Trust me I’ve al-…” She closed her mouth.
“I’ll search for a spell, don’t worry.” Twilight lifted her hoof up next to Celestia, tears started to arrived just after Cely started to cry.
“No, Twily. You won’t, I can’t be with you anymo-…” Celestia jumped a little when Twilight started yelling.
“I don’t allow you to say that, Celestia. You can’t run away from me, we’ve already talked about this! I want to be with you, I love you.”
“B-but…” She paused while crying. “You have to be happy too. Like your friends, you don’t need to have pity on m-…”
“Excuse me?! Pity on you?! What do you think I am? An hypocrite? I AM happy when I am with YOU! I don’t care if you can’t have babies! We’ll adopt, that’s all!”

Twilight kissed Celestia and throwed the results of Celestia’s blood test.
