Easy answer for that one. Save scumming. Game makes you rely on it too much due to odds for a shot missing or hitting arent that random.

If you try to shoot but you miss, then reload the save of that move and do the same move, you will just keep missing. I tried it even when I had a 80% chance of a hit, I just kept missing after 6 reloads.
