This is true, but in the medical field specifically they are required to be as up-front, blunt, and clear as possible.
It’s actually detrimental to use ‘soft’ terminology, because when you have someone who is emotionally thinking this poorly, saying she has to gain “some” weight for example places subjective concepts. It’s important to remove those notions and give a factual baseline so that the patient can begin to grasp that they must see (and accept) the world as is, rather than the way they want to.

IOW, the way this doctor is speaking is true-to-life, at least based on my experiences and observations. And I’ve no doubt the artist (who themselves said struggled with anorexia and it seems other mental stresses) is relating to personal experiences as well.
But yes, it can be heartbreaking and crushing. I knew it would be.
Once she’s a little more stable, psychological experts can turn to the encouraging soft terms, but right now Sunny is so critical she’s dying.

All right. I had gathered around, but I thought that he would at least soften the pill a bit. I know that he’s stating the facts, but this was like putting a sledgehammer to her mental state. Poor girl had a breakthrough!

I know that she couldn’t leave so soon, but why isn’t this hospital offering psychological support? You don’t go to a drug addict, and straight up tell him he has to kick his habit right there and then. Or someone who just survived a plane crash that they’ll get over it, because they’ll need to transport them to another hospital for their own safety?
Not exactly the same. You are right if she were simply underweight. Being anorexically close to death means the medical doctor has to do what he can to save her life.
Hospitals with attached mental health wards will offer psychological support, and will see to it that counseling is accessible. Right now, she’s in a dire physical state that needs emergency stabilization first; he’s just stating the facts. The same is true for someone who is morbidly obese to the point they are or nearly are bedridden.
