Honestly, it wouldn’t surprise me if someone on the art team in charge of making the thumbnails intentionally made it suggestive just to see what would happen (I mean, the episode is called “Filly Filling”), and either none of the checkers noticed, or none cared to call them on their bluff (perhaps they even found it funny).
Then, when it was released publicly, it could’ve been a complaint, or maybe an executive or something saw it and wasn’t amused.

Y’know, looking at this more closely, you can tell what a rush-job this was - just look at the sloppy outlines around each character (I think it’s especially apparent around Hitch’s ear). There’s also the fact that Sparky seems less sharp than Hitch and (especially) Sunny.
