Ascending to alicornhood through a profound understanding of love during her childhood, akin to Twilight’s journey with friendship, she was chosen by Queen Celestia to rule over the Crystal Empire, a role confirmed by the Crystal Heart after a persistent struggle.
Her reign has been marked by numerous victories over the recurring threat of King Sombra, who persists as a danger beyond the protective borders of the Crystal Mountains. In a battle against him, Cadance sustained injuries while safeguarding the Crystal Heart, resulting in a transformative change to her cutie mark, symbolizing her unwavering loyalty to the empire. The acceptance of her rule by the Crystal Heart deepened her connection to the land and its inhabitants, heightening the impact of her responsibilities.
Amidst Crystal Rose’s Princessing festival, the resurgence of King Sombra triggered a traumatic incident where Lovely Promise, in a surge of unexpected magic, freed her sister. King Sombra’s ominous warning resonated, prompting a challenging decision by the family, including Cadance. Crystal Rose embarked on rigorous magical and royal studies to prepare against King Sombra’s potential return. Despite Lovely Promise’s initial magical promise, her uncontrollable powers post-Sombra resulted in a separation in studies and family gatherings, a decision that burdens Cadance’s conscience.
In the face of these adversities, Queen Cadance persists in showering her love and dedication upon her children and the Crystal Empire. Navigating the intricate responsibilities of rulership, she maintains an unwavering commitment to the well-being of her family and subjects, gracefully confronting the complexities that unfold in her realm.

You don’t necessarily need to do that, but you can if that’s the way you wanna go about it. But the ruler of an empire is usually an Emperor/Empress (or sometimes King-Emperor/Queen-Empress, if they want to denote their royal descent, though Emperor/Empress is a higher title than King/Queen anyway).
The British Empire was always ruled by a king or queen, not emperor or empress. Use of the terms isn’t a science.
