Not always. While red is the most common color for Sith lightsabers, they’ve been confirmed to use just about every other existing color.  
The only reason they use red is because a lightsaber’s color is decided by the crystal used to focus the blade. Red crystals are made using Sith Alchemy, as naturally-occuring crystals aren’t always available.
