Isn't Doctor Horse his official name by way of merchandise, though? Specifically the CCG. That's "Doctor" whereas the one in S07E20 is "Dr.", if you need a finer line for it.
Not that it has any real bearing, but the name in A Health of Information feels like an error where someone on the art team didn't realise that "Doctor Horse" was a preexisting character. Or maybe they decided to swap him out, but forgot to change the script. Something for Jim's next Twitter Q&A, maybe.
