Maybe it was initially a slur used to attack trans people. But it gradually became something of a joke referring mostly towards fictional characters who look like they're of genders opposite to theirs. Definitions can expand and change over time right? A term can have multiple meanings itself. It all depends on context and how it's used. Any term can be misuse in abusive ways.
