That’s simple - he doesn’t turn into dog - he turn into his double from EQG, who happened to be a dog. His double can’t exactly stop being born as a dog, so he will always turn into a dog by default. Universe doesn’t care about girlfriends or awkwardness. Unless Twilight modifies the Portal to not transform anyone into their doubles, of course.

The pufferfish version makes him look more squishy and adorable. That’s probably all there is to it.
 
What I really want Hasbro to answer for is the hypothetical situation in which he grows up, gets a girlfriend, and goes to the Canterlot High world. Would he transform into an older dog, making things awkward? Or would the dictate that his girlfriend cannot be anything other than a dragon who also becomes a dog just to doge the controversy they set for themselves?

What this tells me is the magic only bastardizes dragons. Human world? Becomes a dog. Seaponies? Puffer fish.
