Methinks they wouldn’t get far… even if she’s bound as she is… but yeah, that’s the only way I can imagine she’d have her horn unharmed, and tied as she is: Someone knows her ‘fetish’.

True enough, one could be more caring of her on a personal level, with the other being more professional, tending to “aftercare” with a semi-sadistic nature which is used to delivering pain, but also in making it part of a “greater experience”.

Fair enough, but they’re not mutually exclusive. She could desire either or both depending on the occasion.
 
And I can’t help but imagine it’s not always the same one(s), and that different consorts might treat her differently after. One might, as you suggest, be disturbed, while another might place gentle, reverent kisses on her bruises while tending to her wounds, honored to be allowed to give her both the hurting and the healing that she needs.

It’s more a matter of “Being put so close to death, despite being what could very well qualify as “immortal”, through a severe degree of physical trauma.
 
As for helping? Well, I prefer the idea of the “protagonist” thinking that her masochistic nature is messed up as all hell, and after helping her to tend to her fantasies of being absolutely beaten to hell & back, he cares enough for her to legitimately try helping her cover up wounds, rubbing cotton-swabs with alcohol on them into her skin, being gentle with her, etc.. This is as opposed to just being a guy who’d beat the hell out of her and barely hear her say “thank you…” before leaving to a suspiciously empty castle.

I think she might also get a thrill out of being made to do debase, degrade, or defile herself, or being otherwise humiliated. I can see her yearning to be forced to do things her high station would otherwise forbid her from doing. It’s an interesting sort of irony, really, the thought of her wanting to be treated like a whore so she can forget for a bit that she’s a princess.
 
And as for tending her wounds? Yeah, for more reasons than one. Not only is pissing off an alicorn a bad idea, but if you just walk away without so much as putting a band-aid on her, and the sun doesn’t come up tomorrow, you’re gonna feel like a total jackass.

THAT SAID, when it comes to what she’d like to do and have done to her, I can imagine that she’d be the type who’d like to get “close to the point of death” in terms of masochism… considering how old she is, she’d probably get a rush out of being beaten, let alone bound and gaged…
 
… You’d have to be a fool, though, not to help tend to her wounds after a rigorous session with her.
