Thier is a fair chance at some point in anyones life they might be injured or have an accident and find themselves sedated/passed out/asleep via anesthesia in the hospital for at least half a day, and in which case it’s likely family will visit. I know almost everyone in my family has had this at least once in their lives including me, 2 week stay.
 
So.. if you get a NSFW tattoo that is likely visible in this state.. you should really just accept family will likely see it somehow someday.
