Well, they measure time of harmony in periods, which means at least that they lost it multiple times, and at most that is is not default state.
Everything she has done to Twilight was to prepare her for the worst, to make her a good leader and to be able to stand strong in the worst case scenario.
She enigineered such scenarios from real threats where all Equestria was the stake. She did nothing to teach Twilight friendship before throwing her into NMM. She considered overcoming her own obstacle (the test) more important than actually saving the country from Sombra. She does not just prepares Twilight, she thinks that she must do it in swim or sink way while simultaneously believing that if Twilight fails anywhere, everything would be lost anyway.

We have no data about this over a thousand years period except that the villanous takeover parade which happened during the show was proclaimed to be the longest period of harmony in recent years.
We also have no data on the fact that the ponies ever lost their harmony in the time period she was ruling, so in this case no one is in the right or wrong.
Also, her actions against the parade suggest that she is either supernaturally cunning, or indeed careles, or just entirely relies on Friendship and Harmony fixing everything in mysterious ways, such as throwing Twilight into NMM without any preparation, or derailing the Crystal Empire mission by turning it into a test and refusing to make a failsafe.
