This is just a pony that resembles and claims to be Pear Butter. This doesn’t mean that they are relatives, since she, unlike Pear Butter, is not a member of the Pear (later Apple) Family.
 
Also, as shown below, Pear Butter’s cutie mark is a jar:  
 
Furthermore, Pear Butter is still deceased in the show and comics, as well as people’s headcanons (all of them). However, the only way she can still be alive is if a portal takes them to the past and that the past is before giving birth.

Upon closer inspection, you can tell she isn’t Pear Butter. Note the heart in her cutie mark. That, plus this pony’s eyes are green; Pear Butter’s are turquoise.

Is that Pear Butter or is just an unnamed pony reassembling Pear Butter?
