Not all wishes Atamanna makes are terrible for the wisher. in fact a well worded wish can give the wisher exactly what the wisher wants (though Atamanna tends to ignore word lawyers who want to manipulate her power). This however, is not an example of a well said wish, but instead a chance to use her powers to benefit someone… while screwing over someone else. Atamanna cares not what Snips and Snails do with this newfound power, but sees it as an opportunity to spread a little more trouble. Sure, whoever the boys kiss will be made to believe that they love their kisser, but odds are that they would rather be with someone else. When it comes to the “karmic backlash” of Atamanna’s wishes, it’s fine just so long as someone is left worse off by results of the wish. Usually, at least.
For future reference, while this ask is considered NSFW, any wishes or plot related events involving foals will always be safe for work, and will go no further than what is presented here. What you choose to write about and how you portray it says everything about who the author is.
Of course I morally object to things like mutilation or nonconsensual sadism - or anything nonconsensual, really. However, I’m also aware that these are fictional settings. In the context of a fictional setting, a nonconsensual scene says literally nothing about the author - it only makes the character who commits it a villain. I honestly don’t think many people think of Dainn and his ilk as anything else but villains, myself included. I am writing a Post-FoE Resistance story, by the way.
It’s pointless to talk about the moral side, because in countries with a population of 2.8 billion people, the legal age of consent is 14 years or less.

Let me say it again: This is not an Equestria Trainers’ Society image. It’s a Wish World image.
But okay, since you want to talk about EqTS so badly, let me humor you for a bit.
First of all, your criteria for narrative elegance seem to be massively warped. How is such an ass-pull “elegant”? Not to mention all the issues and unfortunate implications that come with it, but I’ll get to that in a moment.
It seems you want to make a moral point here, not one about narrativity or world-building (despite the fact that these are all purely fictional settings, but okay). So let me give you my two cents regarding these age regression potions. If you mentally regress an adult to the stage of a child and then fuck him, it’s still child abuse. It doesn’t matter if the adult gave consent beforehand, because a child cannot give consent, and therefore also cannot reaffirm it. It also doesn’t really matter if you change the person back afterwards, because retroactive consent is not consent (for pretty much the same reason why you can’t retroactively withdraw consent either). I’m honestly surprised Clocktower would even touch that kind of stuff. Clocktower stands out among all these settings in that it’s the only one that deliberately doesn’t want to be a rape fantasy and thus at least tries to depict all interactions as consensual. This little detail makes me think that the Clocktower authors don’t have a good understanding of consent though. And these are just the problems that come from using the potions to let an adult regress to the mental stage of a child. Don’t even get me started on the issues that come with the possibility of using the potions the other way around! (Then again, I’m not sure if they’re used in this second way in Clocktower, since I don’t read it.)
On the other hand, Equestria Trainers’ Society doesn’t involve any foalcon at all as far as I know, and no age regression either. If you claim that EqTS involves any kind of foalcon, I want to see evidence for that.

Reading these lines, I have a mental comparison of 3 universes, in 2 of which you have fanfics.
Clocktower Society
Fall of Equestria
Equestria Trainers’ Society.
The most frightening of them is Fall of Equestria, where the foals are mutilated from the very early age by the removal of the horn or wounds of the wings. Sexual exploitation is likely to exist without serious restrictions.
Equestria Trainers’ Society. - probably not massively, but individual cases of the exploitation of foals (or for example deer) reaching puberty. Justify yourself with local laws on the age of sexual consent, which are different in countries and in different states.
Clocktower Society - the most elegant solution - magical potions of raising and lowering the age, and it is possible to change the age of only the body or body and mind.
