I imagine there would be some drawbacks and limitations to magic. It wouldn’t necessarily be a quick and perfect solution and medical science would still be important (if anything science could coexist with magic, like you suggested earlier).
 
The Alicorn Amulet suggested gender swap exists (but also more or less implied that it’s a rare spell that many unicorns don’t know). So I more or less treated gender swap spells as a rare ability and that for the most part, the general spell for it is temporary but with certain potions and medication, it can last longer and even be permanent.

reverting sex change to how it was previously is literally impossible. The surgeon is forced to de facto do a sex reassignment surgery again but on the other side, and no you are not going to have you old penis back, it will be a ftm surgery in that case. Also you cannot do it while you are still healing from the previous surgery. Spring basically accepted her new body, and was her right to renounce to be reassigned back to male. Also considering that it would have never brought back to how she was before.

I have an opinion, and this is the reason why i implied srs for Spring instead of using magic/alchemy. If being able to modify the body of someone using magic is possible Equestria would have been a Tsimisce paradise(for those who don’t know what a Tsimisce is, is a clan of vampires form the Vampire The Masquerade franchise. They have te power to fleshcrafting), Basically everyone would be a pegasus, everyone would add some appendices to grab object easily, everyone would add more muscles, and there would be no ugly or fat ponies, and so on. This beacause magic is fast and accessible and can let you change your appearence. Also there would not be phisicians and earth pony phisicians in general, this because medical science would be for the most part unneccessary due to magic (Tempest would have had no problems to have her horn back). Quite not the case right?  
So, eccept for some magic that can help heal fast, for the most part magic that can change someone’s anatomy is limited in time (for instance the magic that turned the mane 6 in breezies), so surgery and plastic surgery to have permantent results are the way to go.

So do I. Mine’s a ftm unicorn who is an alchemist. And since alchemy involves changing one thing into another, I figured that maybe he could use his own magic to do this process for himself (and for other ponies who want to transition).

That’s usually what I headcanon. Temporary versions of that spell can help same sex couples have children and a permanent one can help trans ponies transition.

Do they use magic for sex reassignment so their new organs function for reproduction? Because a temporary version of that spell might explain some magical lesbian spawn.
