"Shot" and related tags

There appear to be a rather varied set of tags applying to characters being shot, and pictures of particular body regions. Shot, gunshot, and bullet hole all refer to being shot, but it’s somewhat unclear on the first two what the tense should be (possibly both?) More of a headache is “headshot” being aliased to portrait while head shot is a mix of being shot in the head and portraits, but butt shot refers to butt portraits.
 
I would like some ideas on how to handle this in a sane manner.

I think it should be “headshot (wound)” and interlinking descriptions, unless it breaks some policy on description ratings, for being shot in the head; “injection” for “shot” in the medical colloquial sense, and “buckshot” for images depicting ammunition consisting of load of multiple pellets intended for use in smoothbore guns, if there’s any. “shot” itself probably should be only used for images actually containing the word, and possibly not even then.
 
Tags that are blocked from being added to images and display a message describing what tags to actually use when.
 
It’s not even just tense; “shot” can be past tense of “shoot”, pellet ammo, medical injection (both the fluid, syringe, and the process of being injected), and figurative meaning of “shooting” a photocamera, hence “headshot” aliased to “portrait”.
