Phallus size being related to frame size is a pornography trope, and like all pornography, it is a fiction. If Rarity and Spike were “able to get the job done” in Kilala’s universe, then the most obvious thing most obviously occurred and Tab A fits into Slot B just fine, thank you very much (and often, too, as their multiple children suggest.)
