You can see how America-centric the show is when Applejack mentions the need to raise money for Granny Smith’s hip replacement. But despite that, I agree that Celestia would have implemented universal healthcare long ago.
With a kind and compassionate leader like Celestia, who isn’t motivated solely by profit at the cost of all else, I’m pretty sure Universal Healthcare would have been the standard in Equestria for so long that it’d be taken for granted by its citizenry.
