Stockings and heels on nurses were popularized by japan and their nurses stereotype: Caring and sweet but also very sexy by the uniform (or that’s what they tend to look).
Izzy may have found the outfit somwhere trying to be the nurse of someone not knowing the true meaning of what she’s currently wearing, a thing that she could do for real, a sweet mistake

Why in the world is the chat quickly turning into a ‘let’s criticize Izzy for wearing a clearly fake nurse outfit since she isn’t a trained nurse with a medical degree’ chat? Do you all do that to anyone that wears a fake nurse or doctor outfit?

Especially you. Izzy doesn’t have a medical degree, sure, but that doesn’t mean you have to be mean to her. She is just playing doctor, not trying to actually be a doctor (that’s a different field for a different type of pony).

I don’t understand the design, why would a nurse be wearing thigh high stockings and a choker? It makes the design come off as sexualized, but the context and dialogue seems merely whimsical, like a child.

Honestly, I don’t think Izzy would be the type of pony to even attempt to practice actual surgery on another pony. She would rather create gifts for other ponies instead.
