In the fandom’s early days there was a fanfic called “Bittersweet” where Pinkie Pie dies from diabetes. It used to very well known but I don’t think it’s as well known these days.

Look, Pinkie I want to tell you this from a place of love, I’m not trying to be mean, but you really need better care of yourself. Surviving off sweets and inedible materials is hazardous to your health and if you won’t listen to anypony else, at least listen to yourself, the contents of that ceramic plate and exorbitant amounts of sugar are killing you as we speak. The only reason you’ve lasted this long is because of your fast metabolism and because of cartoon physics among other things.
