I’d say you managed to incorporate their wishes into your style really well! Cadance is big, sure, but it blends well with the way you draw the anatomy. All in all the effect is still realistic, if slightly larger than life.
As for the rest, Cadance and Shining’s loving expressions and the sweet pose make for an adorable picture!

It’s certainly bigger than I would usually draw this sort of thing and I was struggling a bit with making it work with my understanding of the anatomy. But that’s what the commissioner wanted so that’s what they got.
Well, like I stated before, when looking at the sizes foals are in the show, even Flurry heart herself, Princess Cadence’s tummy size in this image leads me to think that there is more than one foal in there. So that’s honestly what I was getting at, not so much how the realistic horse pregnancy looks.
That’s just my perspective though.
True, and it might be an effect of the angle (Cady is laying down in the picture; the horse is standing). I’m used to seeing people draw pregnant ponies like they eat bowling balls instead of more realistically, but something about the belly looks just a bit off.
For me it almost looks like the first picture I showed you.

I’m aware they can grow big and the time it takes before foaling, it’s just that this particular image makes Cady look a bit too big, especially when compared to the images below.
It is not unusual to see a pregnant mare with a big belly. It is so easy to find it on google.

Horse pregnancies typically last 11 months, and the size of the mare’s tummy doesn’t get this size with one foal. And that’s based of real life horses. Seeing how small foals are in the show, I’d say there’s more than one foal in her tummy right now.
