Many are assuming those are bullet holes. But we also saw her back in the last panel. No exit wounds. I would argue that these are actually burn marks, like from a cigarette.
 
Because bullets are always going through the target, right?  
There was never, ever, EVER situation when bullets did stuck in somebody after being shot.

Bullets don’t always exit the target.
 
Chances are she had an extremely painful process to have the slugs removed and insides repaired (probably by twilight).
 
I actually like this interaction so far. Maybe Spike and Rarity will have scar showcasing (if Spike has any scars on him beyond the mark on his head). But I’m also all for honest sympathy that isn’t just “OH! HOT BOD!”

Looks like spike needs to help kiss the boo boo’s to make them feel better. hehehe
 
All kidding aside, I can see why it would seem such an embarrassment to have such a wound and scars. For her, it would be a form of imperfection, not just outwards, but functional, causing her to never be able to bare children of her own. That kind of injury would caused psychological harm.

We (but not Spike) have seen the other side two (maybe 3?) strips ago and there didn’t appear to be any exit wounds so that seems unlikely.
 
Though honestly that’s not really better, as I’m not too familiar with how scarring like this works I can’t tell if they were ever removed or not

Injury wise, that is some serious shit. Scar wise, not nearly as bad, but looking at them I say the bullets pierced her all the way.
