He made it up in his head. I mean “Crystal Hoof” is already clear that it’s a poor attempt at ponylike names with what Thorax knows, what with his own name being a body part.

Who was the original?
There might not be an original. We’ve all just assumed they can only take on forms they’ve already seen. This form could be totally fictitious.
 
It apparently didn’t even occur to him that he could change and walk in the city undetected. Likely he just made the appearance up on the spot.
