Actually, Zecora is. She’s the pony everypony goes to when they’re sick and in need of a cure. Even in terms of animal care Zecora probably has Fluttershy beat. When Big Mac lost his voice, nopony asked Fluttershy for medical knowledge, they just went to Zecora.
Second to Zecora is Twilight Sparkle, who has been known to read up into equestrian illnesses and has actually worked towards curing ponies of magical ailments. When Apple Bloom was sick with the Cutie-Pox, Twilight was the first pony Applejack went to. It’s only unfortunate Equestrian Medicine did not yet know the cure.
Fluttershy can fix a broken mouse leg, but when dealing with illnesses or worse conditions, she’s entirely reliant on alternative treatments such as acupuncture and aroma therapy. She has all the medical knowledge of a mom with some first aid training.
Really, the only reason Fluttershy is always shoehorned into the healer class is because people want her to be super nice and friendly, because apparently you can’t be nice and friendly if you’re a mech pilot.
